Appointments
The clinic is open to see patients every Wednesday from 1-4pm except the 4th week of the month we are open on Teusday from 1-4pm. We see patients on a scheduled basis only and are not a "walk in" or urgent care clinic. If you call the clinic at other times you may leave a message, but remember, there will not be anyone in the office to recieve the message until our next clinic day, so please do not call with emergencies. For emergencies, dial 911. An appointment will be made for you after you are registered with the clinic and have been accepted based on the financial information you have provided.
If you are registering for the first time, please remember to bring in financial info that gives evidence of your income for the last 6 months. You can expect a short interview process during which time, our staff will be reviewing your financial and social information in order to qualify you to recieve free health care. There are some forms to be filled out as well, two of these forms can be downloaded below, filled out and brought in on your initial visit.
Documents such as your last tax return, or pay stubs will suffice for financial qualification.
Additionally, please have a form of picture ID w/ you upon registration and each time you come in for an office visit. You must renew your registration every 6 months at the clinic.
